Pediatric GI Issues in Hyderabad: What Parents Need to Know
Pediatric GI Issues in Hyderabad: What Parents Need to Know

Children's digestive systems are more sensitive, more resilient, and more complex than adults' — and when they go wrong, the consequences affect the entire family. From recurrent tummy pain that keeps a child home from school to chronic constipation that causes distress at every meal to bloody stools that terrify a parent at midnight — pediatric GI problems are among the most common health concerns in Hyderabad families.

At KIMS Advanced Gastroenterology & Endoscopy — KAGE — KAGE, KIMS Hospitals, Secunderabad, Secunderabad, our gastroenterologists — including Dr. Sreekanth Appasani and our team, supported by dedicated pediatric endoscopic equipment including a Pediatric Endoscope and Colonoscope — provide expert diagnostic and therapeutic care for children with GI disorders.

This guide covers the most common pediatric GI issues in Hyderabad, what to watch for, and when to seek specialist help.

Why Pediatric GI Issues Deserve Specialist Attention

Children are not small adults — their gut physiology, response to illness, and tolerance for investigation differ significantly. Many pediatric GI symptoms are dismissed or mis-attributed to 'growing pains' or 'stress', delaying accurate diagnosis. Conditions including IBD (increasingly diagnosed in children and adolescents in India), celiac disease, H. pylori infection, and eosinophilic esophagitis require specialist endoscopic evaluation for correct diagnosis — and can significantly impact a child's growth, nutrition, and quality of life if missed.

Common Pediatric GI Problems in Hyderabad

            1. Recurrent Abdominal Pain

The most common GI complaint in children aged 5–15. While functional (non-organic) causes are most common — including functional abdominal pain, anxiety-related gut symptoms, and IBS — recurrent abdominal pain warrants evaluation to exclude organic causes including H. pylori gastritis, celiac disease, IBD, and constipation-related pain.

              2. Constipation

Chronic constipation in children — hard, infrequent, or painful stools — is extremely prevalent in Hyderabad. Causes include low dietary fibre (processed snacks, refined foods in school lunchboxes), inadequate water intake, and psychological factors (toilet anxiety, school refusal to use toilets). Chronic constipation causes abdominal pain, loss of appetite, soiling, and emotional distress. Management includes dietary fiber increase, hydration, toilet training, and in some cases medication.

                3. Acute Diarrhea and Gastroenteritis

Viral gastroenteritis (rotavirus, norovirus) and bacterial infections (Salmonella, Shigella, Campylobacter) from contaminated water, street food, or school canteen food are extremely common in Hyderabad's children. Rotavirus is the most common cause of severe diarrhea and dehydration in young Indian children — rotavirus vaccination dramatically reduces its incidence.

                  4. Celiac Disease

An immune-mediated intolerance to gluten (found in wheat, barley, and rye) that damages the small intestinal lining — causing malabsorption, diarrhea, poor weight gain, fatigue, and anaemia. Celiac disease is significantly underdiagnosed in India. Diagnosis requires blood tests (anti-tTG antibodies) and upper GI endoscopy with duodenal biopsy — available at KAGE.

                    5. Pediatric IBD

Crohn's disease and ulcerative colitis are increasingly diagnosed in children and adolescents in urban India including Hyderabad. Pediatric IBD presents with chronic diarrhea, rectal bleeding, abdominal pain, weight loss, and growth failure. Early diagnosis and treatment — including biologics — is critical for preventing long-term bowel damage and growth impairment.

                      6. H. pylori Infection in Children

                      H. pylori infection is common in Indian children due to the high population prevalence. It causes chronic gastritis, peptic ulcer disease, and iron deficiency anaemia in children. It is diagnosed by urea breath test or stool antigen test — non-invasive options available at KAGE — and treated with antibiotic eradication therapy.

                        7. Eosinophilic Esophagitis (EoE)

An increasingly recognized condition — particularly in children with allergies or asthma — characterized by eosinophil accumulation in the esophageal lining causing swallowing difficulty, food getting stuck, and chest pain. Diagnosed by endoscopy with esophageal biopsies at KAGE.

                          Pediatric Endoscopy at KAGE

KAGE is equipped with a dedicated Pediatric Endoscope and Colonoscope — smaller diameter instruments specifically designed for children — allowing safe, precise upper GI and lower GI endoscopy in pediatric patients. All pediatric procedures at KAGE, KIMS Hospitals, Secunderabad, Secunderabad, are performed under appropriate sedation with parental support.

When Should a Child See a Gastroenterologist?

                            • Recurrent abdominal pain lasting more than 4 weeks that disrupts school attendance
                            • Blood in the stool — any amount, at any age
                            • Chronic diarrhea lasting more than 2 weeks
                            • Poor weight gain or weight loss in a child who is eating
                            • Persistent vomiting or spitting up beyond infancy
                            • Difficulty swallowing or food refusal beyond infancy
                            • Jaundice in a child
                            • Persistent bloating or abdominal distension

                                  Frequently Asked Questions

Endoscopy can be performed at any age when clinically indicated — including in infants and toddlers. KAGE uses dedicated pediatric endoscopes specifically designed for children's smaller anatomy, under appropriate sedation. The decision to perform endoscopy is based on clinical necessity and is carefully discussed with parents.

Daily abdominal pain coinciding with school time may be functional — related to school anxiety or avoidance — but organic causes must be excluded before assuming a psychological etiology. A consultation at KAGE will evaluate for H. pylori, constipation, celiac disease, or early IBD — and if these are excluded, provide guidance on managing functional abdominal pain.

Celiac disease is diagnosed by: blood tests for anti-tissue transglutaminase (anti-tTG) antibodies; followed by upper GI endoscopy with duodenal biopsy confirming villous atrophy. Both are available at KAGE. The diagnosis is confirmed while the child is on a gluten-containing diet — do not start a gluten-free diet before testing, as this gives false-negative results.

Pediatric IBD management shares the same treatment principles as adult IBD — 5-ASA agents, immunomodulators, and biologics — but with specific considerations for growth monitoring, nutritional support (exclusive enteral nutrition is a first-line option for pediatric Crohn's), school attendance, and the psychological impact of chronic illness on a developing child. KAGE's IBD Clinic provides dedicated care for pediatric IBD patients.
